What To Bring To Your Appointment
We ask that you bring the following documentation to your consultation:
- Health insurance card
- Physician referral form (public medical insurance patients)
- List of prescribed and over-the-counter medications
- Reports of previous visits to doctors and inpatient treatments
- Heart record card, diabetes record card, pacemaker and cardiac pacing record card, etc.
Appointment Cancellations
If you cannot keep an appointment you have made with us, please let us know two days in advance; we may be able to assign the appointment time to other patients.
